A DP FMEA proving trials document is available from Vassallo Associates as part of the comprehensive Dynamic Positioning test services on offer and is a requirement by classification societies for all DP equipment class 2 and 3 vessels. We write an easy-to-read document that describes the entire vessel DP system (propulsion, power and control) with the inclusion of simplified drawings. It has sufficient detail to enable a reader to understand how a system works and to correctly assess the failure mode. All documents comply with the requirements of the classification society and IMCA guidelines.

Most Active Systems Will Fail

FMEA is a technique to look at all the ways a process or equipment can fail, then knowing the potential failures, the following can be considered:

  • What are the potential consequences of a failure?
  • What measures are required to avoid failures of the entire system or process?

On DP vessels, the FMEA is a means of proving the redundancy concept of a vessel. It needs to be continuously reviewed and updated to include all changes and modifications to the vessel’s DP systems throughout the life of the vessel, to remain valid. DP systems include control systems, power generation/distribution and propulsion. Other equipment also has a bearing on the DP system, and things like ventilation, air cooling and safety systems are also included.

The FMEA should be easy to understand but have sufficient detail and simplified drawings so that a reader can understand all the systems described, and correctly assess their failure modes.

When the FMEA is first completed, it is tested for accuracy with a trials programme called the proving trials, followed by trials every year, to ensure the vessel remains as described in the FMEA.

After the DP FMEA has been written, a set of tests to check its accuracy is complied, called the proving trials. This is a comprehensive test programme of all systems and equipment analysed in the FMEA. Each tests shows the expected outcome, and if the actual outcome is different, the document analysis and the equipment is question is checked. Then a modification is made to either the document, if the result is satisfactory and complies with the general guidelines, or a change is made to the equipment.

These trials are repeated if the vessels undergoes a major modification, with additional tests for new equipment as appropriate.

Updating

Vassallo Associates offers a comprehensive review of the FMEA after each 5 yearly periodic trials, making amendments as necessary to keep the document in date. It can also be updated on request, as equipment is added or removed.
 

IMCA DP trials and Assurance Practitioner

At least one person accredited by IMCA in the position of DP trials and assurance practitioner is on the team writing an FMEA, and usually more.

The objective of the FMEA is to identify, through desktop analysis and supporting DP FMEA proving trials, any single failures of the DP system of the vessel that may lead to a loss of position and/or heading by ‘drift off’ or ‘drive off’.

DP Operations Manual

All DP vessels that need to have an FMEA also require a vessel specific DP Operations Manual. Vassallo Associates can write and supply this document either at the same time as an FMEA, or independently.
 

Operational Activity Planning

All DP vessels require an operational activity planning system, consisting of a Critical Activity Mode (CAM) and Activity Specific Operating Guidelines (ASOG) documents, the latter we provide in an editable format, to enable modification as the vessel changes jobs, clients and area of operation. The document also gives advice on Task Appropriate Mode setup.
 

Annual DP Trials

DP trials which are required annually, within 3 months, plus or minus, of the original FMEA proving trials anniversary date.

Periodic DP trials, which are conducted within 5 years of the original FMEA proving trials, and every five years subsequently.

A DP FMEA is vital because it systematically identifies potential design failures, assesses their impact, and prioritises corrective actions, ensuring product reliability, safety, and customer satisfaction.
